Reporting An Injury – Step by Step Guide

  1. Immediately after an incident, submit an OC Sport Injury Report Form

  • Can be submitted by a Club Director, Ride Leader, Parent/Guardian, Team Manager, Organizer, First Responder or Injured Athlete

  • Must be submitted immediately after incident (OC must file with insurance provider within 30 days)

  1. Have medical expenses as a result of an incident during a sanctioned activity? Contact the OC Office to obtain a Sport Accident Claim Form.

  • Claim form must be filed by Ontario Cycling with the insurance provider within 90 days of incident.

  • Only medical expenses which occurred in Canada are covered, and coverage is applicable after Provincial Health Care Coverage (Ex. OHIP) and any existing medical benefit policy the member has access to has been exhausted.

  • Even if you have the above coverage items, it is recommended to submit an accident claim form within the 90-day period to ensure outstanding medical expenses may be eligible to be claimed.

  1. Email the completed Sport Accident Claim Form (Including Physician’s Statement) to Ontario Cycling.

  • We will confirm the incident took place during a sanctioned activity and submit the form on your behalf.

  1. A representative from the insurance company will reach out to the injured member.

  • Someone from the insurance company will reach out to the injured member for next steps (usually within 5-10 business days).

If at any time during the process you have any questions, feedback or need assistance, don’t hesitate to contact our office. We are here to assist you through this process.
